Patient Journey Coordinator - Band 4
An exceptional opportunity has emerged for a Patient Journey Coordinator to become a vital member of the Pod 2, Respiratory Administrative team. The successful candidate will collaborate within a multi-disciplinary Point of Delivery (POD) team, providing essential support in the proactive oversight of the patient journey from initial referral through to discharge, ensuring that every step of the process is managed efficiently. This role will also serve as the primary contact for patients concerning all administrative matters related to their care pathway.
Main ResponsibilitiesThe individual in this role will act as the main point of contact for all administrative concerns associated with the patients' care journey. Working closely with a diverse team, the Coordinator will bring expertise in the proactive management of the patient journey, ensuring a seamless experience from referral to discharge. Responsibilities include:
- Supervising and delegating tasks within the POD team.
- Ensuring high levels of satisfaction for both patients and clinicians by being an approachable, customer-focused, and knowledgeable resource.
- Implementing a consistent approach across the organization by utilizing Standard Operating Procedures (SOPs) to guarantee that all functions of the role are executed accurately and within designated timeframes.
