The successful candidate will be employed by Greenwich Health, the GP Federation in Greenwich, working for Valentine Health Partnership, part of Valentine Primary Care Network. This role is ideal for individuals who are passionate about delivering high-quality patient care and are looking to develop their skills in a dynamic and supportive environment.
Key Responsibilities- Signpost patients to relevant services based on their health needs, ensuring they receive the best possible care.
- Support vulnerable patients in communicating with healthcare providers, promoting their independence and well-being.
- Liaise with ancillary departments and other care providers to facilitate appointments, procedures, treatment plans, clinical letters, drug information, and results.
- Act as the point of contact between the Practice and other healthcare providers, raising queries as requested by clinicians.
- Assist with social outreach initiatives, including homeless initiatives, food banks, domestic violence programs, and AGE UK for vulnerable patients.
- Contribute to QOF and other quality initiatives, ensuring the highest standards of care are maintained.
- Train registrars (trainee doctors) on the eRS system, promoting knowledge sharing and best practices.
