We are seeking a highly skilled and compassionate Social Worker to join our Hospital Pathways Team in Adult Social Care. As a key member of our team, you will play a vital role in empowering adults to live independently and make informed decisions about their care.
Our team works closely with NHS colleagues to facilitate safe and timely discharge, enabling individuals to return to their own homes and regain independence. We are committed to delivering exceptional care that enhances outcomes for our community and provides a supportive and inclusive environment for our staff to grow their careers.
About YouWe are looking for an individual with outstanding communication skills, a compassionate manner, and the ability to effectively manage high levels of emotional distress. You will have a strong understanding of relevant legislation, including the Care Act and Mental Capacity Act, as well as social work theories.
As a qualified Social Worker with current registration with Social Work England and eligibility to work in the UK, you will have a minimum of 2 years post-qualifying experience working in adult services, assessing and supporting individuals with diverse needs. You will also have a strong ability to communicate complex social issues clearly to both patients and professionals.
