About the Role
We are seeking a skilled and compassionate Community Nurse to join our team at the Royal Devon University Healthcare NHS Foundation Trust. As a key member of our Community Nursing team, you will play a vital role in delivering high-quality patient care and support in the community setting.
Key Responsibilities
- Assess, provide, and review nursing care, advice, and information to patients and carers in the home environment, alternative care settings, and clinic environments.
- Maintain patients in their preferred place of care, enabling them to maximize their independence and optimize their quality of life.
- Work under the direction of the Nurse Specialist (Community) and the Community Nurse Team Lead, utilizing your skills and knowledge as a Registered Nurse.
- Engage in autonomous lone working and remote working without direct supervision.
- Supervise, teach, and involve the development of peers and other relevant team members, carers, and clients.
Working Pattern
The Community Nurse will work a set number of hours per week, spread across three days. The team covers a 7-day service, including weekends.
