Job Title: Positive Practice Worker – Heatherwood Court

We are seeking a dedicated and compassionate Positive Practice Worker to join our team at Heatherwood Court Hospital, Pontypridd. As a Positive Practice Worker, you will play a vital role in delivering person-centred care to individuals with complex mental health and behavioural needs.

Key Responsibilities:
  • Work collaboratively with clinical teams to ensure adherence to person-centred plans and promote a focus on person-centred outcomes.
  • Act as a guide and advisor for staff, providing structured support and guidance on positive behaviour support plans.
  • Link with established clinical teams to ensure prescribed interventions are delivered and care plans are co-produced with service users and their families.
  • Collect and monitor outcome data on a daily basis to inform care planning and delivery.
Requirements:
  • Interest in and commitment to the practice of clinical psychology as an applied science or career aspiration in a related discipline.
  • Awareness of current policy agendas relating to person-centred practice, values-based practice, and reducing restrictive interventions.
  • Previous experience in working with individuals with mental health and learning disabilities is desirable.
