Job Title: Health, Work and Wellbeing Nurse Advisor

We are seeking a motivated and enthusiastic Nurse Advisor to join our team at the Transformation Unit. This is a unique opportunity to work in a dynamic and supportive environment, contributing to the delivery of high-quality nursing care and making a real difference in the lives of our patients.

About the Role

The Nurse Advisor will work closely with a range of colleagues in primary and secondary health, social care, and the voluntary sector to provide high-quality evidenced-based support and advice to patients, carers, families, and staff across the interface pathway. The successful candidate will have a flexible approach to this role, with the ability to work a variety of shifts, including weekends and Bank Holidays.

Main Responsibilities
  • Contribute to the delivery of high-quality nursing care through receiving and responding to callers accessing the SPA line.
  • Assess health care needs and ensure referral to the appropriate health care provider in accordance with clinical need and signpost to social and voluntary sector.
  • Work as an autonomous practitioner and provide a holistic approach to deliver a high standard of clinical expertise in the process of telephone triage assessment, planning treatment education, and advice given to patients, carers, and other health and social care professionals.
  • Supervise and lead the staff within the single point of access in their absence, promoting positive staff behaviour and facilitating the delivery of treatment to individuals, groups, and communities.
Requirements
  • Registered Nurse with current registration.
  • Educated to post-graduate level (academic level 6) or demonstrable knowledge, training, and experience.
  • Specialist training experience, courses at diploma level.
  • Telephone triage.
  • Previous Primary care or community experience.
  • Relevant extensive post-registration experience.
  • Broad understanding of Sheffield community services.
  • Knowledge of specialist long-term condition management.
  • Knowledge of the management of acute episodes of illness.
  • Line management experience and knowledge of Human Resources processes.
  • Patient assessment skills and experience.
  • Adult protection.
  • Enhanced verbal articulation, listening, and interpersonal skills.
  • Flexible to meet the needs of the service.
  • Knowledge of SystmOne.
