About the Role

We are seeking a highly skilled and experienced Community Learning Disability Nurse to join our Intensive Support Team on a fixed-term contract to cover maternity leave. As a key member of our team, you will play a vital role in providing high-quality care and support to individuals with learning disabilities and complex health needs.

Key Responsibilities
  • Work collaboratively with the Intensive Support Team, Learning Disability Leads, and commissioners to support the use and management of the Dynamic Support Register.
  • Identify individuals who require or would benefit from a Care Treatment Review and develop a plan to meet their needs.
  • Contribute to the Dynamic Support At Risk Register for adults who are at risk of being admitted to hospital.
  • Promote national learning disability agendas, including Transforming Care, Positive Behaviour Support, and mental health recovery models.
  • Be aware of the need to protect individuals from abuse and harm in line with local safeguarding policies and procedures.
Requirements
  • Registered Nurse Learning Disability/RMN/Dip OT/Dip SW with current registration with NMC/HCPC/SWE.
  • BSc in community health studies or equivalent qualification.
  • Evidence of post-registration training/qualifications and continuous professional development.
  • Able to meet the travel requirements of the role.
  • Good knowledge of working with people with a learning disability and their families and carers.
  • Good knowledge of making reasonable adjustments for people with learning disabilities and vulnerabilities.
  • A good level of knowledge of current policies and legislations that impact upon people with a learning disability.
  • At least two years post-registration experience working with people with complex health needs in the field of Learning Disability and/or Mental Health.
