About the Role

We are seeking a highly skilled and compassionate Service Coordinator to join our team at Net-Worx (2001) Limited. As a Service Coordinator, you will play a vital role in supporting customers who have complex needs and/or are experiencing multiple disadvantage.

Key Responsibilities
  • Co-ordinate access to a wide range of services, including healthcare, education, and social services.
  • Lead on the development and implementation of individualized support plans.
  • Work closely with customers, their families, and other stakeholders to ensure seamless service delivery.
  • Monitor and evaluate the effectiveness of support plans, making adjustments as needed.
  • Collaborate with internal teams, including housing, healthcare, and social services, to ensure a comprehensive approach to customer support.
Requirements
  • Proven experience in a similar role, preferably in a housing or social services setting.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with customers and stakeholders.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
  • Ability to work independently and as part of a team, with a flexible approach to working hours.
  • Knowledge of relevant laws, policies, and procedures, including the Mental Health Act and the Care Act.
