We are seeking an experienced and dedicated Community Support Worker to join our Specialist Outreach Recovery Team. As a key member of our team, you will play a vital role in delivering high-quality support services to individuals with complex mental health needs.

Key Responsibilities
  • Provide person-centered support to individuals with complex mental health needs, promoting recovery and well-being.
  • Work collaboratively with the multidisciplinary team to develop and implement individualized care plans.
  • Conduct regular assessments and reviews to ensure the effectiveness of support plans.
  • Develop and maintain strong relationships with service users, families, and carers.
  • Participate in team meetings and contribute to the development of new initiatives and projects.
Requirements
  • 2 years' experience of working with individuals with mental health problems.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Knowledge of mental health legislation and policies.
  • Ability to travel across Plymouth and other geographical areas.
