Job Summary:

We are seeking a skilled Registered Nurse to join our team as a Nurse Care Coordinator. The successful candidate will work in the community, providing high-quality care to clients and building customized care plans.

Key Responsibilities:

  • Conduct initial patient assessments, monitor vital signs, symptoms, and well-being, and evaluate changes to care or medical requirements.
  • Carry out a full range of registered nurse duties, including assessment, planning, and implementation and evaluation of the patient's condition and care, with support from staff members in service delivery.
  • Show compassion, respect, and maintain the privacy and dignity of patients.
  • Demonstrate procedures, train, and supervise staff undertaking care or nurse training.
  • Prioritize nursing care, recognize deterioration, changes, reactions, conduct physical exams to assess patients, and alert senior staff or healthcare professionals immediately.
  • Perform wound care, ensure wounds are cleaned and dressed as required, and comply with infection control policies and procedures.
  • Take patients' temperatures, blood pressure, blood samples, set up drips and blood transfusions if required.
  • Prescribe assistive medical devices, related treatments, adjust, and administer medications as needed.
  • Communicate effectively with clients, families, and healthcare professionals.
  • Maintain effective registration with the Nursing and Midwifery Council (NMC) and ensure professional portfolios are kept and updated in accordance with NMC guidelines.

Requirements:

  • Registered Nurse qualification and current NMC registration.
  • Ability to work in a community setting and commute to and from an office environment.
  • Excellent communication and interpersonal skills.
  • Ability to prioritize tasks and manage time effectively.
  • Knowledge of infection control policies and procedures.
