Mental Health Care Coordinator

Hunter AHP is seeking a skilled and dedicated CPN Care Coordinator to join our Adult team in the North West region. As a key member of our multidisciplinary team, you will be responsible for coordinating, commissioning, and delivering individual packages of care for a caseload of service users within Community Mental Health Teams (CMHTs).
Key Responsibilities- Undertake initial assessments of new referrals to the service, ensuring comprehensive and accurate evaluations of service users' needs.
- Develop and implement personalized care plans, working closely with service users, their families, and other healthcare professionals to ensure seamless care coordination.
- Provide high-quality patient care, demonstrating a commitment to delivering exceptional outcomes and experiences.
- Collaborate with multidisciplinary teams, including psychiatrists, psychologists, social workers, and other healthcare professionals, to ensure comprehensive and coordinated care.
- Support the duty system, ensuring effective communication and handovers to ensure continuity of care.
- Current and valid registration with the Nursing and Midwifery Council (NMC).
- Proven experience within a CPN recovery role, with a strong understanding of mental health care principles and practices.
- Strong communication and interpersonal skills, with the ability to build rapport with service users, families, and healthcare professionals.
- Ability to work independently and as part of a multidisciplinary team, with a flexible and adaptable approach to meet the needs of service users.
- Commitment to ongoing professional development, staying up-to-date with best practices and evidence-based care.
