Position Overview

The role of a Community Healthcare Associate is pivotal in delivering exceptional care to individuals experiencing acute health challenges within their residences. This innovative service operates across various regions, providing an alternative to traditional hospital care and facilitating quicker transitions from hospital to home.

This service employs a blend of in-person consultations, telephonic and video communications, alongside advanced digital monitoring technologies, ensuring patients can recuperate safely in their own environments.

The service encompasses six distinct pathways, addressing a broad spectrum of health conditions, including but not limited to frailty, respiratory issues, heart failure, and recovery from surgical procedures. Our dedicated team administers a comprehensive array of acute healthcare interventions within the home, while also vigilantly monitoring health conditions and vital signs for any signs of deterioration.

In this role, you will acquire a diverse skill set in acute care within a community framework. You will actively participate in a multidisciplinary approach, engaging in collaborative rounds and working alongside a team of consultants, advanced care practitioners, nurses, and therapists to formulate effective treatment strategies that significantly enhance the well-being of local residents.

Key Responsibilities

Community Healthcare Associate - Band 4

Your responsibilities will include:

  • Delivering acute care to patients with intricate and specialized needs within the community.
  • Collaborating with the multidisciplinary team to create, implement, and assess care plans.
  • Evaluating the health and psychosocial requirements of patients in both hospital and community settings.
  • Designing and executing treatment plans utilizing a variety of services to address patient needs.
  • Employing a broad range of clinical competencies to support the ongoing management of patients in their home environment, including but not limited to venepuncture, cannulation, and wound care.
  • Effectively communicating and networking to ensure high-quality, responsive, safe, and compassionate care for patients, their families, and caregivers.

The successful candidate will collaborate with the team at Southmead hospital and system partners. A valid full UK driving license and access to an insured vehicle are essential for this role, with travel expenses reimbursed.

Essential Qualifications and Skills

  • Registered Nursing Associate on the NMC register.
  • Nursing Associate Foundation Degree/Level 5 Apprenticeship.
  • GCSE Grade A-C in Maths & English or equivalent.

Desirable Qualifications

  • Teaching experience or qualification.
  • Evidence of ongoing personal and professional development.
