Job Summary

We are seeking a highly skilled and compassionate Mental Health Practitioner to join our Intensive Outreach Team in South East Essex. As a key member of our team, you will work closely with service users who require intensive support and care, providing a range of services to promote their mental health and well-being.

Main Duties
  • Assess, plan, deliver, and evaluate care for service users referred to the Intensive Outreach Team, including risk assessment and management.
  • Manage a caseload of patients as the named key worker/care coordinator/lead clinician, working collaboratively with service users and their carers to develop personalized care plans.
  • Provide psychosocial support and education to service users and their carers, promoting empowerment and self-management of mental health conditions.
  • Work within a collaborative framework with other agencies and disciplines to ensure seamless care and support for service users.
  • Participate in mental health promotion and education, providing advice and information to clients and carers on maintaining good mental health and preventing relapse.
