Job Title: Care Coordinator

We're seeking a skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in ensuring the smooth delivery of our care services, working closely with our care workers and clients to provide exceptional support and care.

Key Responsibilities:

  • Manage care worker rotas to ensure efficient and effective service delivery
  • Build and maintain strong relationships with clients and care workers
  • Provide ongoing support and guidance to care workers to ensure high-quality care
  • Collaborate with the care team to identify and address any issues or concerns

Requirements:

  • Experience in a care coordination or related role

  • Ability to work independently and as part of a team

Working Hours:

Monday to Friday, 9am - 5pm, with additional on-call duties on a rota basis
