We are seeking an experienced nurse or HCP to join our team as a Clinical Lead in the South Hub based at Abingdon Community Hospital. Key Responsibilities- Lead the operational service delivery within the community, working closely with acute sector, and Social Service colleagues to adapt working style, flex capacity, and implement escalation measures to meet immediate and long-term pressures.

- Lead and be accountable for the implementation of performance indicators within the team.

- Lead the development and implementation of service-specific policies, procedures, and protocols.
- Advanced knowledge and understanding of health and social services, including their access criteria and availability.
- Ability to work in partnership with acute, primary, and community HCPs with system navigation and managing the inbound community referrals for Oxford Health.
- Strong problem-solving skills and ability to lead the development of local operational policy to support best practice and the delivery of quality service.
- Ability to work within all Trust policies and procedures, and your professional code of conduct.
