Position Overview

We are seeking a dedicated Adult's Social Worker to become a vital member of our Mental Health Team.

Role Requirements

This position necessitates a recognized Social Work Qualification along with a minimum of two years of post-qualification experience.

Team Dynamics

The Mental Health Team operates as a multidisciplinary unit, delivering essential support to individuals facing mental health challenges and their families. Key responsibilities include participating in S117 aftercare planning meetings, conducting placement evaluations, facilitating hospital discharges, and executing safeguarding procedures in accordance with the Care Act 2014. A strength-based intervention approach is crucial to customize care packages that promote maximum independence for each client.

Candidate Profile

Applicants should possess experience in a mental health environment, whether in community settings or forensic contexts. A degree in Social Work (Degree/DipSW/CQSW) and at least two years of relevant experience are mandatory for consideration. A valid UK driving license and access to a vehicle are also required.
