About the Role

We are seeking a dedicated and experienced Social Worker to join our multi-disciplinary team in the Isle of Dogs Neighborhood Mental Health Team area of Tower Hamlets. As a key member of our team, you will play a vital role in providing high-quality care and support to adults aged 18-65 with severe and enduring mental health problems.

Key Responsibilities
  • Conduct comprehensive assessments and develop individualized care plans to support clients in achieving their recovery goals.
  • Work collaboratively with health and social care professionals, as well as other agencies, to ensure a coordinated approach to care.
  • Provide expert advice and guidance to clients, their families, and carers on mental health issues and available support services.
  • Develop and maintain effective relationships with clients, their families, and carers to ensure a person-centered approach to care.
  • Participate in multidisciplinary team meetings and contribute to the development of care plans and strategies.
Requirements
  • A recognized social work qualification and registration with Social Work England.
  • Experience of working in a community mental health setting, with a focus on adult social care.
  • Excellent team player with strong communication and interpersonal skills.
  • Ability to work with clients, their families, and carers from diverse backgrounds and with varying levels of need.
  • Knowledge of relevant legislation, policies, and procedures, including the Care Programme Approach, Mental Health Act, Mental Capacity Act, and Care Act.
