We are seeking a highly skilled Clinical PCN Pharmacist to join our team at the One Network Alliance, a Tower Hamlets Primary Care Network (PCN). As a key member of our team, you will play a vital role in implementing an effective medicine management system within our PCN practices.
Main Responsibilities:- Develop and implement a comprehensive medicine management strategy to achieve cost-effective prescribing and improve patient outcomes.
- Work as an independent prescriber, providing advice and support to patients and colleagues on medication-related matters.
- Conduct clinical medication reviews, producing recommendations for senior clinical pharmacists, nurses, and GPs on prescribing and monitoring.
- Collaborate with care home staff to enhance the safety of medicines ordering and administration.
- Attend and refer patients to multidisciplinary case conferences to ensure comprehensive care.
- Identify high-risk patients through pre-prepared practice computer searches and develop targeted interventions to mitigate risks.
- Review and optimize the use of medicines associated with unplanned hospital admissions and readmissions.
- Manage a caseload of patients with common/minor/self-limiting ailments, providing signposting to community pharmacy and referrals to GPs or other healthcare professionals as needed.
- Provide patient-facing clinics and a telephone helpline for medication-related queries and concerns.
- Reconcile medicines following discharge from hospitals, intermediate care, and care homes, ensuring continuity of supply to high-risk patients.
- Answer medicine-related enquiries from GPs, network staff, and other healthcare teams, suggesting solutions and providing follow-up for patients.
- Ensure timely referrals to appropriate healthcare professionals for patients with pathology results, common/minor ailments, acute conditions, and long-term condition reviews.
- Develop and implement a repeat prescribing policy within each PCN practice, managing the reauthorisation process and ensuring patients have necessary monitoring tests.
- Contribute pharmaceutical advice for new services with medicinal components, analyzing and presenting medicines data to inform decision-making.
- Undertake clinical audits of prescribing, providing feedback and implementing changes with practice teams.
- Implement changes to medicines resulting from MHRA alerts, product withdrawals, and local/national guidance.
- Monitor practice prescribing against the local health economy's RAG list, making recommendations for hospital doctors and shared care.
- Provide education and training on therapeutics and medicines optimisation to primary healthcare teams.
- Ensure practices comply with CQC standards related to medicines.
- Support public health campaigns, providing specialist knowledge on available programs.
- Qualified member of the GPhC with an undergraduate degree in pharmacy.
- Experienced in the PCN Clinical Pharmacist role, with a clear understanding of its needs in primary care prescribing.
- Completed the CPPE Primary Care Education Pathway or working towards it.
- Qualified as an independent prescriber or working towards qualification.
- Interest in completing the advanced clinical pharmacist pathway.
Up to £55,677 per annum, depending on skills and experience.
