Job Description:

The Bridge Support Community Rehab is a Third Sector provider with two rehab houses set in Kingston. Our service aims to deliver specific recovery-focused interventions to individuals with severe and enduring mental illness, enabling them to maintain and develop their daily living skills and understand and develop strategies to manage their symptoms.

This will include medication management and monitoring physical wellbeing. As an Occupational Therapist, you will work closely with our staff to ensure effective risk management, the development of personal goals and skills to maximize daily living, and support for move-on to further independent living.

Our team provides community care to individuals with severe and enduring mental health problems who have been discharged from inpatient care. Clinical interventions will be intensive but time-limited, with a focus on recovery and assessment for further placements.

You will work closely with our Care Coordinator responsible for the clients. There is an expectation that clinical interventions will have an outcome. Assessments will be carried out using the Model of Human Occupation, and treatments will be determined and carried out in the form of individual or group intervention.

The Occupational Therapist for this service is managed by the Community Rehab Team based at Springfield Hospital, and the OT is professionally managed by the Lead OT for Wandsworth.

Key Responsibilities:

  • Deliver specific recovery-focused interventions to individuals with severe and enduring mental illness
  • Work closely with staff to ensure effective risk management and development of personal goals and skills
  • Support individuals in move-on to further independent living
  • Carry out assessments using the Model of Human Occupation and determine treatments
