Job Description:
The Bridge Support Community Rehab (Bridge 24) is a Third Sector provider with two rehab houses set in Kingston. Our service aims to deliver specific recovery-focused interventions to individuals with severe and enduring mental illness, enabling them to maintain and develop their daily living skills and understand and develop strategies to manage their symptoms.
We provide community care to individuals with severe and enduring mental health problems who have been discharged from inpatient care. Clinical interventions are intensive but time-limited, with a focus on recovery and assessment for further placements. You will work closely with the Care Coordinator responsible for the clients, ensuring effective risk management and the development of personal goals and skills to maximize daily living.
Key Responsibilities:
- Deliver occupational therapy interventions to individuals with severe and enduring mental illness
- Work closely with the Care Coordinator to ensure effective risk management and personal goal development
- Conduct assessments using the Model of Human Occupation and develop individualized treatment plans
- Provide group and individual interventions to support daily living skills and symptom management
Requirements:
- Qualified Occupational Therapist
