Job Title: Support Worker - Complex Care Specialist

Job Summary:

We are seeking a dedicated and compassionate Support Worker to join our team at Libertatem Healthcare Group. As a Support Worker, you will play a vital role in providing high-quality care and support to individuals with complex health needs, ensuring their comfort, safety, and well-being.

Key Responsibilities:

  • Delivering Personalized Care: Provide tailored care and support to clients with complex health conditions, promoting their independence and dignity.
  • Assisting with Daily Living Activities: Assist clients with personal hygiene, mobility, and meal preparation, ensuring their physical and emotional needs are met.
  • Medication Administration and Health Monitoring: Administer medication and monitor clients' health conditions, reporting any changes or concerns to healthcare professionals.
  • Emotional Support and Companionship: Provide emotional support and companionship to clients, promoting their mental well-being and social interaction.
  • Record Keeping and Reporting: Maintain accurate records of care provided and report any changes in clients' conditions to healthcare professionals and family members.
  • Collaboration and Communication: Collaborate with healthcare professionals, family members, and other stakeholders to ensure comprehensive care and support.

Requirements:

  • Previous Experience: Previous experience in a similar role, preferably in complex care, is essential.
  • Communication and Interpersonal Skills: Excellent communication and interpersonal skills are required to build strong relationships with clients, families, and healthcare professionals.
  • Compassion and Empathy: A compassionate, patient, and empathetic nature is essential for providing high-quality care and support.
  • Flexibility and Teamwork: Ability to work independently and as part of a team, with flexibility to work the specified hours.
