Job Role Description

About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at Rose Farm House, a specialist care service for adults with intellectual disabilities and complex needs. As a Support Worker, you will play a vital role in providing high-quality care and support to our residents, promoting their independence, dignity, and well-being.

Key Responsibilities

  • Provide person-centred care and support to residents with complex needs, including those who use alternative communication methods.
  • Develop and maintain positive relationships with residents, their families, and other stakeholders.
  • Participate in and promote a team approach to care, working collaboratively with colleagues to achieve excellent outcomes.
  • Contribute to the development and implementation of individual care plans, ensuring that residents' needs are met and their rights are respected.
  • Monitor and report any changes in residents' physical or emotional well-being, taking prompt action to address any concerns.

Requirements

  • Experience of working in a care or support role, preferably in a similar setting.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with residents, colleagues, and other stakeholders.
  • A commitment to providing high-quality care and support, with a focus on promoting independence, dignity, and well-being.
  • A willingness to participate in ongoing training and professional development, to ensure that you have the skills and knowledge needed to excel in this role.
