Job Summary

We are seeking an experienced Care Coordinator to join our team, responsible for assessing and reviewing the needs of adults with disabilities or health conditions.

Key Responsibilities
  • Conduct comprehensive assessments of individuals' strengths and needs, identifying areas for support and development.
  • Develop and implement personalized support plans, working closely with individuals, their families, and healthcare professionals.
  • Review and update support plans regularly, ensuring they remain effective and meet the evolving needs of service users.
  • Collaborate with healthcare colleagues, voluntary sector organizations, and other partner agencies to ensure seamless service delivery.
  • Apply statutory legal frameworks and policies, including the Care Act 2014 and the Mental Capacity Act 2005, to ensure compliance and best practice.
Requirements
  • Proven experience in care coordination, assessment, and support planning.
  • Knowledge of adult social care policies and legislation, including the Care Act 2014 and the Mental Capacity Act 2005.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to work at pace in challenging situations, making sound decisions quickly to support individuals in crisis.
  • Experience of working in a fast-paced, dynamic environment, with a strong focus on customer-centered care.
