Key Responsibilities

Under the guidance of senior members of the Community Children's Nursing Service, you will:

  • Facilitate the transition of children and young people from hospital to home.
  • Provide specialized knowledge to address the diverse needs of patients and their families in various community and hospital environments, serving as a resource for paediatric expertise and offering guidance to other professionals while promoting evidence-based practices.
  • Manage a clinical caseload within your competency scope.
  • Oversee the ongoing assessment, care planning, implementation, and evaluation of specialized nursing care in collaboration with children, young people, and their families.
  • Collaborate with the broader multidisciplinary team.
  • Ensure safeguarding practices are upheld.
  • Maintain documentation in accordance with NMC standards.
  • Execute clinical responsibilities, including medicines management, blood sampling, and administration of medications (IV, IM, S/C, and CVADs), wound management, and care for children and young people with complex health needs.
  • Demonstrate effective teamwork while also being capable of working independently in patients' homes, collaborating with children, young people, their families, and multidisciplinary teams.
