Job Overview

We are seeking a highly motivated and experienced Paediatric Community Nurse to join our team in the Community Children's Nursing Service. As a key member of our team, you will be responsible for providing high-quality care to babies, children, and young people in the community setting.

Main Duties
  • Support the transition of babies, children, and young people from hospital to home.
  • Provide specialist knowledge to meet the diverse range of needs of patients and their families in a variety of community and hospital settings.
  • Hold a clinical caseload within your scope of competency.
  • Be responsible for the ongoing assessment, care planning, implementation, and evaluation of outcomes of specialised nursing care in partnership with children, young people, their families.
  • Work in partnership with the wider Multi-disciplinary team.
  • Work within safeguarding policy and best practice.
  • Maintain documentation as per NMC and local requirements.
  • Main clinical responsibilities will be; Oncology care - blood sampling and management of CVAD's. Administration of medications via IV, IM, S/C. Gastrostomy management. Enteral feeding device and stoma management. Respiratory care. Care and management of babies, children, and young people with complex health needs.
  • Be an effective team player with the ability to work autonomously in patients' homes. Working collaboratively with Children/Young people/their families/carers and multidisciplinary teams.

Detailed Job Description and Main Responsibilities

The post holder will be responsible for the on-going assessment and complex care needs, the development, implementation, and evaluation of programs of specialised nursing care for babies, children, young people, and their families/carers in the community. They will provide specialist knowledge to meet the diverse range of needs of patients and their families/carers in a variety of community and hospital settings, acting as a resource of paediatric expertise and providing specialist advice to other professionals, promoting evidence-based practice.

The post holder will work in partnership with the wider Multi-disciplinary team, work within safeguarding policy and best practice, and maintain documentation as per NMC and local requirements.
