Key Responsibilities- Provide comprehensive nursing care to patients in their own homes, including assessment, planning, implementation, and evaluation of care plans.
- Work collaboratively with multidisciplinary teams, including doctors, therapists, and other healthcare professionals, to ensure seamless care delivery.
- Develop and maintain strong relationships with patients, families, and carers to promote patient-centered care and support.
- Conduct regular home visits to assess patient needs, provide education, and promote independence.
- Collaborate with other healthcare professionals to develop and implement care plans that meet the unique needs of each patient.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Registered Nurse (RN1) qualification and current registration with the Nursing and Midwifery Council (NMC).
- Experience in community nursing, preferably in a similar role or setting.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to work autonomously and as part of a team, with a flexible and adaptable approach to meet the changing needs of patients and services.
