About the Role

We are seeking a highly motivated and experienced Community Health Nurse to join our team at Herefordshire and Worcestershire Health and Care NHS Trust. As a key member of our community nursing team, you will play a vital role in providing high-quality patient care and support to individuals in their own homes.

Key Responsibilities
  • Provide comprehensive nursing care to patients in their own homes, including assessment, planning, implementation, and evaluation of care plans.
  • Work collaboratively with multidisciplinary teams, including doctors, therapists, and other healthcare professionals, to ensure seamless care delivery.
  • Develop and maintain strong relationships with patients, families, and carers to promote patient-centered care and support.
  • Conduct regular home visits to assess patient needs, provide education, and promote independence.
  • Collaborate with other healthcare professionals to develop and implement care plans that meet the unique needs of each patient.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Registered Nurse (RN1) qualification and current registration with the Nursing and Midwifery Council (NMC).
  • Experience in community nursing, preferably in a similar role or setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work autonomously and as part of a team, with a flexible and adaptable approach to meet the changing needs of patients and services.
  • Basic IT skills and proficiency in using electronic patient records and other healthcare systems.
