About The Role

We are seeking a highly skilled and experienced Field Care Supervisor to join our North Essex team. As a Field Care Supervisor, you will be responsible for engaging with customers to maximise their independence and achieve individualised goals.

Key Responsibilities
  • Carry out initial assessments, outcome-focused support planning, and subsequent customer reviews in a variety of settings.
  • Work closely with our integrated team of health and social care workers, as well as other external health professionals, to maximise the support available to our customers.
  • Develop and maintain a comprehensive knowledge of local resources to support customers.
About You

To succeed in this role, you will need:

  • A QCF Diploma or NVQ Level 2 in a relevant area, with a willingness to obtain Level 3.
  • Experience of working in care.
  • A driving license and use of a personal vehicle for work purposes.
  • Confidence in travelling around the Essex area.
  • An enhanced DBS Check, which can be arranged through ECL if needed.
  • Exceptional communication skills.
  • The ability to build and develop strong relationships with stakeholders.
  • Solid IT skills.
  • The ability and confidence to work independently and manage your own time.
  • A proactive, adaptable, and customer-focused work ethic.
