This position involves collaborating with a team to provide care within healthcare facilities and residential units, while also fostering strong relationships with colleagues, patients, and other essential stakeholders.

About Our Healthcare Services:

We provide continuous, nurse-led healthcare, including initial assessments upon entry and comprehensive primary care services such as minor surgical procedures, inpatient care, and medication management. We also coordinate with various service providers to ensure integrated care delivery.

The safety of our staff and the individuals we serve, especially those in vulnerable situations, is our utmost priority. We work closely with management to enhance the amount of care delivered on-site, which not only mitigates risks associated with patient transfers but also improves service efficiency and reduces missed appointments.

  • Facilitate the onboarding process for new team members from their first day through the completion of their probationary period.
  • Provide training and mentorship to both clinical and non-clinical staff as needed.
  • Ensure the delivery of high-quality clinical care for all patients under your supervision.
  • Conduct patient assessments, develop care plans, administer treatments, and refer patients to other healthcare professionals or agencies as necessary.
  • Evaluate the fitness of individuals for detention, release, or transfer.
  • Administer healthcare interventions for minor ailments or injuries and take appropriate actions for more serious conditions.
  • Assess substance intoxication and withdrawal, collaborating with relevant support services.
  • Perform initial health screenings for new arrivals, making informed recommendations regarding patient care.
  • Offer professional guidance to Paramedics, Healthcare Assistants, and non-clinical staff to ensure effective healthcare delivery in line with best practices.
  • Maintain accurate and comprehensive clinical documentation.
  • Manage emergency response equipment and respond to all emergency situations during your shift.
  • Lead and participate in primary care clinics focused on wound management.
  • Assist in conducting healthcare audits within the facility.
