We are seeking a motivated and compassionate Clinical Nurse Specialist to join our Hospice Outreach Service team. As part of our multi-disciplinary team, you will provide empowering and compassionate care to individuals with a life-limiting illness.

Your New Role

This is an excellent opportunity for an experienced registered nurse or paramedic to develop their skills and career in palliative and end-of-life care. You will work as part of a responsive and resilient team, providing telephone support, clinic and home-based assessments, and individualized care to patients and their families.

Key Responsibilities

  • Provide advanced communication and presentation skills to patients, families, and healthcare professionals.
  • Work independently as part of a team to deliver high-quality care and support.
  • Develop and implement individualized care plans to improve quality of life and achieve patient-centered goals.
  • Collaborate with other community teams and specialists to optimize patient experience and improve team knowledge and development.
