As a Care Manager in Adult Social Care, you will play a pivotal role in coordinating and overseeing personalized care plans, ensuring that individuals receive the support they need to regain independence and improve their quality of life.
You will work closely with healthcare professionals, families, and the community to deliver holistic care, manage resources, and advocate for the best outcomes for our clients.
Key Responsibilities:- Undertake comprehensive Care Act assessments focusing on the needs of patients open to the CRT.
- Work alongside qualified Social Workers and Occupational Therapists, monitoring and reviewing packages of care to meet individual assessed needs.
- Manage your own caseload, visiting residents within their own homes, care home environments, and hospitals.
- Recent experience of working within Adult Social Care or a similar agency, involving direct contact with members of the public.
- An understanding of the social and legal context within which social care services are delivered.
- Excellent communication skills, advocating for adults to assist them in achieving their outcomes, including signposting and providing information and advice.
