About the Role

We are seeking an experienced and skilled Registered Nurse or Paramedic to join our team as a Community Practitioner - Crisis Response. This is an exciting opportunity to work in a dynamic and fast-paced environment, providing high-quality care to patients in crisis.

Key Responsibilities
  • Undertake clinical assessments and develop treatment plans to meet the complex needs of patients requiring intermediate care and primary care services.
  • Provide safe and effective care to patients, working collaboratively with other healthcare professionals and agencies.
  • Manage and support peers, working on their own initiative and as an effective team member.
  • Develop and maintain knowledge and skills in clinical assessment, diagnosis, and treatment, including non-medical prescribing.
  • Contribute to the development of the service, leading on projects and providing clinical coordination on allocated shifts.
Requirements
  • Registered Nurse Part 1,2 NMC Register or HCPC Paramedic Registration.
  • A minimum of 3 years current experience practicing as band 5 in the Community/Acute environment.
  • Consultation & Diagnostic qualification or working towards.
