Adult Social Care Specialist

This role sits within a specialist Learning Disabilities & Autism Service, providing a social work service to an allocated caseload within the agreed performance management, quality assurance, and budgetary framework.
Key Responsibilities:
- Carry out strength-based assessments and person-centred support planning for customers in the community with complex needs.
- Conduct Mental Capacity Assessments and Court of Protection work.
- Act as a member of a team providing an effective, efficient, and personalised brokerage service to adult service users, carers, and their families.
Requirements:
- Must be a qualified social worker registered with Social Worker England.
