About the Role:
We are seeking an experienced and motivated Heart Failure Nurse Specialist to join our team at Salisbury NHS Foundation Trust. As a key member of our Heart Failure Team, you will play a vital role in providing high-quality patient care and contributing to the development of our services.
Main Responsibilities:
- Provide specialist assessment, investigations, treatment, and advice for patients admitted to hospital with a possible diagnosis of heart failure.
- Comprehensively manage the heart failure patient within the outpatient setting, aiming to improve the patient's quality of life and prevent hospital admissions.
- Arrange and interpret appropriate investigations to ensure evidence-based treatment and management plans are created for all patients.
- Undertake advanced clinical assessment and provide specialist advice, education, and support to patients, their carers, and other healthcare professionals.
- Support and assess competencies for trainee Heart Failure Nurses.

Person Specification:
- At least 2 years' experience as a Heart Failure Nurse Specialist in a UK setting.
- Evidence of continued professional development related to cardiology.
- Knowledge of cardiac pharmacology.
