About the Role

We are seeking a skilled and compassionate Mental Health Practitioner to join our Neighbourhood Mental Health Team in west Birmingham. As a key member of our team, you will work closely with GPs and other primary care professionals to provide high-quality mental health services to individuals in the local community.

Key Responsibilities
  • Provide mental health triage, assessment, and outcomes-based intervention to meet service users' needs.
  • Identify need and best course of advice, support, or intervention, promoting psychosocial wellbeing by identifying and supporting pathways for service users and carers.
  • Ensure robust communication across mental health providers, VCSE organisations, and primary care in accordance with each individual service user's needs.
