We are seeking an experienced Care Coordinator to support our Adults/Disability Services team. As a Care Coordinator, you will play a crucial role in conducting statutory Care Act Assessments for Older People and clients with Physical Disabilities.
Main Responsibilities:
- Conducting statutory Care Act Assessments for Older People and clients with Physical Disabilities
- Managing long-term, complex cases, including hoarding, non-engagers, DoLS challenges, and court of protection work
- Completing six-week reviews
- Conducting Mental Capacity Assessments
- Performing Continuing Healthcare assessments
Requirements:
- Experience working with Older Adults and/or People with Disabilities
- Enhanced skills and knowledge through further training
- Specific experience with the relevant client group
- Innovative and creative approach to improving outcomes for service users
- Highly motivated individual capable of working as part of a team
