About the Role

We are seeking a highly skilled and motivated Adult Nurse to join our team in a busy community setting. As a Community Health Nurse, you will be responsible for providing high-quality, patient-centered care to individuals in their own homes.

Key Responsibilities
  • Conduct holistic assessments and develop individualized care plans for patients.
  • Provide treatment and promote self-care to patients in their own homes.
  • Develop and maintain extensive nursing skills through excellent training opportunities.
  • Work collaboratively with multidisciplinary teams to deliver seamless health and social care services.
  • Stay up-to-date with the latest research and best practices in community nursing.

Requirements
  • Registered Adult Nurse with NMC registration.
  • ENB 998 or equivalent qualification.
  • 1 year post-registration experience in a community setting.
  • Excellent communication and negotiation skills.
  • Demonstrated competence in a range of clinical skills.
