About the Role

We are seeking a highly skilled and motivated Registered General Nurse to join our Urgent Community Response (UCR) Service. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to patients in their homes across Brighton & Hove and East Sussex communities.

Key Responsibilities
  • Deliver holistic assessments, planning, implementing, and reviewing care provision to patients in their homes.
  • Work collaboratively with patients, families, and healthcare professionals to achieve optimal health outcomes.
  • Provide nursing input for patients, including medication management, wound care, catheter care, and identifying deteriorating patients.
  • Participate in the development and implementation of evidence-based care, facilitating staff learning and development.

Requirements
  • Valid full UK driving license and access to a vehicle for work purposes.
  • Experience from a variety of clinical settings, including community and acute care.
  • Knowledge of clinical governance, clinical audit, and clinical supervision, as well as professional codes of conduct.
  • Ability to demonstrate the application of research-based practice and promote independence and choice in undertaking patient care.
