About the Role

We are seeking an experienced and motivated Band 5 nurse to join our Community Nursing Team as a Staff Nurse within the Hambleton & Richmondshire locality.

This is a 7-day service with shift patterns to cover between hours, providing high-quality, holistic care for people in their own homes and residential settings.

Main Responsibilities
  • Provide clinical care and support to the District Nurse Sister and Clinical Lead to our ever-developing teams.
  • Work closely with GP practices and other partners to support patients with complex and high-acuity needs, preventing unnecessary hospital admission.
  • Deliver effective evidence-based care to a defined demographic population with diverse healthcare and social needs.
Requirements
  • Registered as fit to practice by the Nursing and Midwifery Council (NMC).
  • Diploma or Degree in Adult Nursing.
Desirable Qualifications and Experience
  • Teaching and mentoring qualification.
  • Evidence of Post Graduate clinical training/development.
  • Experience working within an acute or community setting.
