About the Role

We are seeking a skilled and compassionate Healthcare Support Worker to join our team at Snowdon ward, a 14-bed neurorehabilitation unit. As a key member of our team, you will play a vital role in maintaining the safety and wellbeing of our patients, delivering personal care while promoting privacy and dignity.

Main Responsibilities

  • Actively promote recovery-focussed care with patients, working in conjunction with other health professionals to achieve set goals.
  • Support patients with their rehabilitation and recovery, respecting their individual needs and rights.
  • Provide personal care for patients where required, using appropriate approved conflict resolution and management of aggression techniques.
  • Accurately assess, describe, and record observations on progress or change in patients' mental and physical state, escalating concerns as necessary.
  • Undertake clinical and environmental risk assessments, documenting key risk areas and seeking guidance from senior staff when required.
  • Provide information on improving health and wellbeing, signposting to other agencies as appropriate.
  • Undertake escort duties to other hospitals or care settings as required.

Person Specification
