We are seeking a highly skilled Advanced Nurse Practitioner to join our Primary Care Network team. As a key member of our clinical team, you will be responsible for delivering high-quality patient care and contributing to the development of our network's health promotion and wellbeing programs.
Main Responsibilities- Develop and implement health promotion and wellbeing programs to improve patient outcomes.
- Manage patients with acute and chronic medical conditions, providing expert advice and care.
- Implement and evaluate individualized treatment plans for patients with chronic diseases.
- Identify and support patients at risk of developing long-term conditions, preventing adverse effects on their health.
- Provide advanced nursing care to patients, adhering to clinical guidelines and best practices.
- Provide wound care, including ulcer and Doppler services.
- Offer specialist clinics, such as ear syringing and contraception services.
- Collect and process pathological specimens.
- Provide travel medicine services and request pathology services as needed.
- Process and interpret pathology and other test results.
- Deliver chronic disease clinics and refer patients to secondary or specialist care as required.

Person Specification- Registered Nurse with the Nursing and Midwifery Council.
- Qualified Independent Nurse Prescriber on the NMC register.
- Masters degree required for qualification post December 2020 (refer to RCN Credentialing for Advanced Level of Nursing Practice).
- Minor illness qualification.
- Experience in venepuncture, ECGs, new patient medicals, chaperone procedure, diabetes, hypertension, CHD, and women's health.
- Experience working as a practice nurse or community nurse.
- Wound care and removal of sutures and staples experience.
