About the Role

We are seeking a motivated and enthusiastic Assistant Practitioner to join our District Nursing team. As a key member of our team, you will provide high-quality care to patients in their own homes, working closely with District Nurses and other healthcare professionals.

Key Responsibilities

  • Deliver nursing care as delegated by the DN team manager/Team Leader, including clinical skills such as catheterisation.
  • Work at all times in accordance with Trust policies, procedures, and protocols specific to the service.
  • Provide sensitive and competent care for patients, demonstrating practical application of your underpinning knowledge.
  • Maintain accurate patient care records in accordance with Trust Guidelines for Records and Record Keeping.
  • Contribute to administration duties required by the service, including telephone enquiries and filing patient notes.

Person Specification

Essential

  • Foundation degree in health and social care or equivalent experience.
  • Previous experience working in a healthcare environment.
  • Previous experience of working in a community setting.

Desirable

  • Experience of working in palliative care.
