**Job Summary**

We are seeking a skilled and motivated Assistant Practitioner to join our District Nursing service team. As a key member of our team, you will provide high-quality care to patients in their homes, working closely with District Nurses and other healthcare professionals.

**Key Responsibilities**

  • Deliver nursing care as delegated by the DN team manager/Team Leader, including clinical skills such as catheterisation.
  • Work at all times in accordance with Trust policies, procedures, and protocols.
  • Provide sensitive and competent care for patients, applying underpinning knowledge and skills.
  • Maintain accurate patient care records, adhering to Trust guidelines.
  • Contribute to administration duties, including telephone enquiries and filing patient notes.
  • Ensure information is accurate and up-to-date for future reference.

**Person Specification**

**Essential**

  • Foundation degree in health and social care or equivalent experience.
  • Previous experience working in a healthcare environment.
  • Previous experience of working in a community setting.

**Desirable**

  • Experience of working in palliative care.
