Job Title: Community Mental Health Nurse

We are seeking a highly skilled and compassionate Community Mental Health Nurse to join our team in providing exceptional care to older adults with mental health needs. As a key member of our multidisciplinary team, you will be responsible for assessing, planning, implementing, and evaluating care plans for a defined caseload of service users.

About the Role

The successful candidate will have a strong background in mental health nursing, with a focus on community care. You will be passionate about delivering person-centered care, working collaboratively with other healthcare professionals, and promoting the well-being of older adults with mental health needs.

Key Responsibilities
  • Assess and develop care plans for a defined caseload of service users
  • Provide high-quality, person-centered care to older adults with mental health needs
  • Work collaboratively with other healthcare professionals to deliver seamless care pathways
  • Contribute to the development and implementation of care policies and procedures
  • Participate in multidisciplinary team meetings and contribute to decision-making processes
Requirements
  • Registered Mental Health Nurse with a valid NMC registration
  • Minimum 2 years post-registration experience in mental health nursing
  • Strong knowledge of mental health legislation and policies
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
