Job Overview

We are excited to announce an opportunity for a Band 3 Community Health Support Worker, offering 12 hours of service per week. The focus of our Community Resource Team is to assist patients in transitioning from hospital to home, providing tailored care packages that cater to individual needs.

The working hours are flexible, spanning from early morning to late evening, including weekends and public holidays. The role requires coverage across various areas to meet service demands.

We seek individuals with a robust work ethic who are committed to delivering high-quality care aligned with our core values. Comprehensive training will be provided, and there are ample opportunities for professional growth, including skill development in areas such as venepuncture and patient observation documentation.

Key Responsibilities

As a Community Health Support Worker, your responsibilities will include assisting patients in maintaining their independence at home through daily living activities such as personal hygiene, meal preparation, and medication management. You will collaborate with a dynamic multidisciplinary team and external organizations to address the needs of individuals.

The role involves being an integral part of the Integrated Multidisciplinary Community Health Care team. You will carry out assigned tasks, primarily involving direct patient care, under the indirect supervision of qualified staff.

Proficiency in Welsh is preferred for this position; however, applications from both Welsh and English speakers are encouraged.

Qualifications

Essential

  • Good general education
  • Qualification Credit Framework Level III (in Care / Health & Social Care) or actively pursuing it

Desirable

  • Basic Food Hygiene certification
  • Evidence of QCF at Level 2

Experience

  • Significant experience in a caring environment
  • Previous/current roles demonstrating interaction with the public in a healthcare context

Desirable

  • Experience in a care setting
  • Experience in a community-based role
