We are seeking a highly motivated and experienced Community Health Nurse to join our team at Betsi Cadwaladr University Health Board. As a Community Health Nurse, you will play a vital role in providing high-quality, patient-centered care to individuals in the community.
Main Responsibilities
Key responsibilities of the role include:
- Assessing, planning, implementing, and evaluating care in the community
- Working as part of the district nursing team to provide clinically effective care
- Developing and maintaining relationships with patients, families, and other healthcare professionals
- Participating in multidisciplinary teams to coordinate care and services
- Providing education and support to patients and families to promote healthy lifestyles and disease prevention

Requirements
To be successful in this role, you will need:
- A degree in nursing or a related field
- Registration with the Nursing and Midwifery Council (NMC)
- Experience of working in a community health setting
- Excellent communication and interpersonal skills
- Able to work independently and as part of a team
Desirable Qualifications and Experience
While not essential, the following qualifications and experience would be advantageous:
- Postgraduate qualifications in a relevant field
- Experience of working with vulnerable populations
- Knowledge of health promotion and disease prevention strategies
Language Requirements
We welcome applications from Welsh speakers and those who are willing to learn Welsh. While not essential, proficiency in Welsh would be an advantage.
