About the RoleThis is an exciting opportunity to work in a dynamic and supportive environment, where you will have the chance to develop your leadership and management skills. You will be responsible for managing a complex and diverse caseload, working to prevent admission to the acute sector and supporting patients in their preferred place of care.
Key Responsibilities- Manage a defined caseload of patients, assessing their care needs and developing, implementing, and evaluating care programs.
- Responsible for maintaining accurate and confidential patient records, adhering to Trust policies.
- Work autonomously, using your clinical expertise to make decisions and take action to improve patient outcomes.
- Collaborate with other healthcare professionals to ensure seamless care delivery and achieve the best possible outcomes for patients.
- Contribute to the development and implementation of innovative solutions to improve patient care and service delivery.
- Registered Nurse - Adult qualification and NMC registration.
- Active on the Mentor Register.
- Evidence of leadership and management development.
- Recognized coaching qualification or willingness to undertake.
- Non-Medical Prescriber/Nurse Prescriber Qualification.
