Community Health Nurse

We are seeking a skilled and dedicated Community Nurse to join our team at NHS Professionals. As a Community Nurse, you will play a vital role in providing high-quality patient care in a community setting.
Key Responsibilities- Assess and monitor patient conditions, arranging diagnostic tests as necessary
- Recognize and respond to deterioration in patient condition, taking appropriate action and escalating concerns when necessary
- Manage emergency situations effectively, displaying excellent analytical ability and sound decision-making skills
- Work collaboratively with patients, families, and healthcare teams to deliver compassionate and person-centered care
- Develop and maintain strong relationships with patients, families, and healthcare professionals to ensure seamless care transitions
