Job Summary
The Complex Care Pathway is a specialized dementia team focused on providing safe and supportive mental health care for adults diagnosed with dementia who are experiencing distress or difficulty due to their illness.
As a Dementia Care Coordinator, you will play a vital role in delivering community-based health interventions for service users. You will work collaboratively to resolve immediate difficulties and improve their situation.
Key Responsibilities
- Provide comprehensive mental health assessments for individuals living with dementia in their own homes and various settings.
- Act as a care coordinator, planning, delivering, and evaluating interventions tailored to each individual's needs.
- Support carers by providing guidance, resources, and emotional support.
- Collaborate with multidisciplinary teams to ensure holistic care for individuals living with dementia.
- Work within the CPA (Care Programme Approach) process to develop and implement individualized care plans.
Essential Requirements
- Current Full Registration with NMC (Mental Health)
- Clean driving licence and access to a vehicle for work purposes
Desirable Requirements
- Experience of working within the CPA process.
- Previous community mental health nursing experience.
- Extensive experience working with individuals living with dementia.
- Knowledge of dementia care best practices.
