Key Responsibilities- Provide holistic and specialist nursing care to patients in their own homes, working within a multi-disciplinary team to achieve optimal health outcomes.
- Develop and implement individualized care plans, taking into account the unique needs and circumstances of each patient.
- Work collaboratively with other healthcare professionals, including therapists, paramedics, and support workers, to ensure seamless care coordination.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
- Provide education and support to patients and their families, promoting health and well-being through health promotion and disease prevention initiatives.
- BSc or equivalent in adult nursing and NMC registration.
- Effective communication skills, written, verbal, and non-verbal, with the ability to adapt communication for individuals with communication difficulties.
- Able to make clinically reasoned judgments when working with a variety of conditions.
- Able to work autonomously, with the ability to know when to seek appropriate advice or guidance from senior staff.
