Clinical Services Coordinator

We are seeking a highly skilled and experienced Clinical Team Leader to join our team at The Disabilities Trust. As a Clinical Team Leader, you will be responsible for managing a team of Registered Nurses and Healthcare Assistants, ensuring the delivery of high-quality, holistic care to our service users.
Key Responsibilities- Lead the nursing team in developing and implementing high standards of nursing care, providing support and guidance to colleagues and relatives.
- Take responsibility for a service user group, consulting and liaising with the multi-disciplinary team, service users, and family/carers to ensure effective care delivery.
- Assess the impact of policy implementation on care delivery and make recommendations for improvement.
- Plan, develop, implement, and evaluate individualized care plans using evidence-based guidelines.
- Produce accurate, contemporaneous, and complete records of service user consultations, consistent with legislation, policies, and procedures.
- Ensure all clinical staff receive formal documented supervision and clinical supervision in line with professional and statutory requirements.
- Identify training needs and support staff in their development, promoting a learning environment that encourages collaboration and knowledge sharing.
- Provide educational support to patients, carers, families, and colleagues, sharing knowledge and expertise through formal and informal teaching.
- Maintain an understanding of and act in accordance with all Trust policies and procedures.
- Registered Nurse or degree-level qualification.
- Experience in specialist palliative or neurological care, depending on service requirement.
- Proven skills in leading, building, and influencing in a clinical setting.
- Knowledge of current legislation within the independent care sector.
- Demonstrable experience of practicing in a self-directed and highly motivated way.
- Effective verbal and written communication skills, including the ability to communicate difficult messages to service users and their families.
- Negotiation and conflict management skills.
- Able to effectively manage change and manage performance.
- An ability to provide teaching and mentorship in a clinical setting.
